ROBERT LAWRENCE, MD, a 19-year member of the Allergy Immunology Department of Northwest Permanente, PC, is also a member of the American Academy of Allergy, Asthma and Immunology. KEN ETTINGER, MD, is Chief of Allergy Immunology, and Physician-In-Charge of the East Interstate Medical Clinic in Portland, Oregon. Abilify is a new drug available for prescription, but was trialed as early as 1995 in Japan when it was discoverd by the Otsuka Pharmaceutical Company Ltd. Successful testing led to it being trialed in Europe in 2000. Initially the drug was thought to be most effective in treating children with early onset BiPolar disorder manic depression ; . Latterly the drug has produced impressive results in treating Schizophrenia in adults as well having known mood stabilizing properties. Abilify is an atypical antipsychotic like the well known Clozzril and Olanzapine medications. However, its mechanism of action is subtly different. The main theory regarding the cause of Schizophrena is that the affected brain has too much of the neurotransmitter Dopamine. Drugs like the above affect what is known as `receptors' that when acted upon determine the level of the neurotransmitter, that is particular to that receptor. Hence anti-psychotics affect the dopamine receptors to lower the level. Abilify is novel because it has the unique ability to increase or lower the dopamine levels should the existing level be too high or too low. These unprecedented properties have led to Abilify being called a `next generation atypical anti-psychotic.' As a bonus, unlike its contempories it affects the serotonin 5HTIA receptors which makes Abilify effective against Panic Disorders and general anxiety. One of the great benifits of using this drug is that the patient's problems are not exacerbated by weight gain. This is a major problem for patients taking the other atypicals. However, as with every medication there are some side effects. The most common are: headache, nausea, vomiting, insomia and dizziness. It should be known that most patients do not suffer these symtoms to a great extent and moreover, these side effects are likely to pass and zoloft.

Clozaril emedicine HPV-associated Anal Intraepithelial Neoplasia and Anal Cancer in HIV-infected, Men Who Have Sex With Men 10. Evidence from several studies shows that HPV-positive men who have sex with men are at increased risk for anal HSILs and might be at increased risk for anal cancer. In view of this evidence, coupled with a recent cost-effectiveness analysis projecting that screening and treatment for anal HSILs provide clinical benefits comparable to other measures to prevent OIs in HIV-infected persons 80 ; , anal cytology screening of HIV-infected men who have sex with men might become a useful preventive measure in the near future. However, further studies of screening and treatment programs for anal HSILs need to be carried out before recommendations for routine anal cytology screening can be made and compazine. This includes all antipsychotics of both generations except clozaril clozapine. There have been no reports of tardive dyskinesia directly attributable to CLOZARIL alone. Nevertheless, it cannot be concluded, without more extended experience, that CLOZARIL is incapable of inducing this syndrome. Both the risk of developing the syndrome and the likelihood that it will become irreversible are believed to increase as the duration of treatment and the total cumulative dose of antipsychotic drugs administered to the patient increase. However, the syndrome can develop, although much less commonly, after relatively brief treatment periods at low doses. There is no known treatment for established cases of tardive dyskinesia, although the syndrome may remit, partially or completely, if antipsychotic drug treatment is withdrawn. Antipsychotic drug treatment, itself, however, may suppress or partially suppress ; the signs and symptoms of the syndrome and thereby may possibly mask the underlying process. The effect that symptom suppression has upon the long-term course of the syndrome is unknown. Given these considerations, CLOZARIL should be prescribed in a manner that is most likely to minimize the occurrence of tardive dyskinesia. As with any antipsychotic drug, chronic CLOZARIL use should be reserved for patients who appear to be obtaining substantial benefit from the drug. In such patients, the smallest dose and the shortest duration of treatment should be sought. The need for continued treatment should be reassessed periodically. If signs and symptoms of tardive dyskinesia appear in a patient on CLOZARIL, drug discontinuation should be considered. However, some patients may require treatment with CLOZARIL despite the presence of the syndrome and amitriptyline.

The deprivation of oxygen and glucose leads to energy failure, cell toxicity, accumulation of reactive oxygen species, and eventually cell apoptosis 2 ; . Massive cell death in brain tissues leads to the symptoms and neurological dysfunctions seen in stroke. Studies using mouse and gerbil models of ischemic stroke have demonstrated the loss of phosphatidylcholine PC ; , sphingomyelin, phosphatidylinositol, and cardiolipin, a mitochondrial membrane phospholipid, in brain cells after stroke 3 ; . However, the loss of PC is particular interest because of its important roles in the cell membrane. PC constitutes 50% of the total phospholipids in the cell membrane and, a loss of only 10% can cause cell death 4 ; . The loss of PC compromises the integrity of the cell membrane thereby allowing free and passive flow of ions into the cell. Excessive loss of PC from the cell membrane eventually leads to apoptosis 4 ; . Normally, PC level in the cell membrane is controlled by a balance between its synthesis and its hydrolysis. PC hydrolysis is mediated by three phospholipases namely, PLA2, PLD, and PC-PLC. PLA2 hydrolyzes PC to form free fatty acids FFA ; , including arachidonic acid, and lyso PC. PC-PLC gives phosphocholine and diacylglycerol, which can be further converted to FFA. PLD produces phosphatidic acid and choline. Increases in the activities of PLD, PC-PLC, and PLA2 have being shown in cells following ischemia 1 ; . The metabolism of the free fatty acids formed as a result of PC hydrolysis increases the formation of reactive oxygen species in the cell, which peroxidize lipids to form aldehydes. Aldehydes can then react with the thiol SH ; group of cell proteins and alters the proteins' functions 5 ; . Synthesis of PC is also impaired in stroke. Synthesis of PC from cytidine triphosphate and phosphocholine is catalyzed by cytidine triphosphate: phosphocholine cytidylyltransferase CCT ; 3 ; . However, CCT, an enzyme for the rate limiting reaction in PC synthesis, is proteolyzed after ischemia 6 ; . Therefore, in ischemia, PC loss is caused by both the reduction in PC synthesis and the increase in its hydrolysis and anafranil. Summarized by Thomas T. Thomas A private psychiatrist with a practice in the East Bay, Randy Bloch, MD, is involved with research on medications for schizophrenia and schizo-affective disorder, bi-polar disorder, major depression, social phobias, and other psychiatric and neurological diseases. Dr. Bloch runs the Affiliated Research Institute ARI ; in Lafayette, California, which participates in clinical trials on both an in-patient and out-patient basis for pharmaceutical companies. At our January 23 meeting, he discussed some of the new medications becoming available for these brain malfunctions. "Currently, we are doing a double-blind study on Seroquel Extended Release, " he said. "Seroquel is an antipsychotic medication with fewer extrapyrimidal side effects--such as muscle stiffness and tardive dyskinesia--than Risperdal or Zyprexa. But Seroquel does require dosing twice a RANDY BLOCH, MD day, which raises the problem of patient compliance. The Extended Release formulation under study would only have to be taken once a day." ARI is one of 30 sites around the country participating in this study, which has been going on for almost a year. All the patients in the study are volunteers and all have schizophrenia. Dr. Bloch establishes this diagnosis by an interview with the patient, examination of his or her medical history, and corroboration by family members. The patient cannot participate if he or she is on Clozarjl or is treatment resistant. "There are different types of schizophrenia, " Dr. Bloch says, "and schizophrenia and schizo-affective disorder are not clearly distinguished. We cannot, for example, take patients who have any symptoms of bipolar disorder." The patient is admitted to the hospital for ten days and taken off any current medications. The patient is put on Seroquel Extended Release at one of a number of fixed dosages, such as 600 or 800 milligrams, or on the placebo. The doctor can only alter the dosage to reduce it if there are side effects. As the study is still ongoing, there is no report as yet on the effectiveness of the Extended Release formulation. "We are also doing several studies on depression, " Dr. Bloch said. "Some are in Phase IV, which is after the drug reaches the market." Clinical trials of medications go through several phases. Of these medications can be confusing: the newer medications are considered "first-line" that is, these are the first antipsychotics recommended ; but are called "second generation" or "atypical." The older medications, though considered "second-line" medications today, are called "firstgeneration" or "typical" because they were discovered first. First-generation antipsychotics like chlorpromazine ; carry significant risk of movement disorders in both the short and long term. Second-generation medications like risperidone ; have less risk of this kind, but carry other serious risks including serious weight gain, development of diabetes mellitus, and abnormal secretion of a hormone called prolactin ; . Use of one secondgeneration medication, clozapine Clozaril ; carries a small but definite risk of bone marrow suppression. Patients on clozapine must have a blood test every other week. Weight gain which increases the risk of diabetes ; is especially pronounced with olanzapine Zyprexa ; and clozapine Clozaril ; . Quetiapine Seroquel ; can cause cataracts in adults; children and adolescents on this medication should have regular ophthalmologic exams. All antipsychotics can be sedating, sometimes to a degree that interferes with optimal functioning and bupropion.

Self destructive and "parasuicidal" behavior, such as cutting oneself with razor blades or taking overdoses, not always with the intent to die. Treatment. While medication may be helpful, some of the most effective treatment is through structured models based on "cognitive behavioral therapy." Provided in both group and individual counseling sessions, cognitive behavior therapies, such as Dialectical Behavior Therapy DBT ; can teach the individual to manage their feelings and replace destructive behaviors with healthier habits through practice, repetition, and supportive relationships. An autopsy was done on February 16, 1999, and a report completed on May 28, 1999, but was not provided to DR VD until June 11, 1999, despite several status requests. The Medical Examiner stated that the toxicology report and outside analysis of JB's heart caused a delay in releasing the report. The report stated that the cause of death was "undetermined" because no toxicological or anatomical cause was demonstrated. Blood work showed positive for valproic acid Depakote ; and clozapine Clozaril ; . Post-mortem blood was not analyzed. The medical expert's report was done by an independent doctor of psychiatry and neurology. The report was based on his review of the following infonnation: witness testimony from JB ' s medical team; medical summary dated February 16, 1999 ; emergency room report dated February 15, 1999; autopsy report; SEVTC death summary; three videotapes recorded on or about August 8, 1998; medication logs; emergency programming records from 1997; laboratory reports; interdisciplinary notes from December 1997 through February 1999; treatment plans; policies and procedures; medical examination reports; quarterly reviews; physician's orders from March 1991 through February 1999; and other documentation provided by DRVD.
